New Patient Registrationregister_now

If you live within our practice area (a detailed map of our practice area is available at reception) please bring proof of your ID and address together with your medical card to the surgery or alternatively we will provide you with a GMS1 to complete.

We recommend that you attend a new patient medical with our nursing staff.

Medical treatment is available from the date of registration. 

Please contact the surgery by telephone or in person to enquire whether you fall into the practice catchment area. We require photo ID and proof of address (utility bill or bank statement which must be no more than 1 month old – we cannot accept mobile telephone bills)
